Vote on a request to accept $1,050 Community Health Grant funds
Introduction & Background: Diabetes continues to be among the leading causes of death in Chatham County. These Community Health Grant funds are provided annually to support a voucher program for medical nutrition therapy and diabetes self-management education.
Discussion & Analysis: Theses funds are to be used to implement a voucher program for Medical Nutrition Therapy appointments and Diabetes Self-Management Education appointments, in coordination with the Orange County Health Department.
