Action Requested: Vote on a request to accept $2,250 Community Health Grant Funds.
Introduction & Background: Orange County Health Department (OCHD) received the Community Health Grant in 2018 and was re-selected in 2019. Chatham County nutrition services, is a sub recipient for this grant.
Discussion & Analysis: Chatham County will be responsible for the following under the grant:
1. Replicate the OCHD voucher program for 100 Medical Nutrition Therapy and 50 Diabetes Self-Management Education appointments to the target population of Chatham County uninsured and underinsured clients ($15 voucher/MNT or DSME unit).
2. Manage tracking of voucher appointments and submit monthly invoices to OCHD for reimbursement.
Total amount to be received= $2,250
How does this relate to the Comprehensive Plan: This voucher program will improve access to nutrition services for those uninsured or underinsured. Increased access to these services can improve the health of the individuals, reducing obesity and chronic disease effects.
Budgetary Impact: No Local Funding requested.
