Abstract

Action Requested: Vote on a request to approve $7,560 Community Focused

Eliminating Health Disparities Initiative Funds.

                     

Introduction & Background: This initiative is a joint effort between Community Care of

North Carolina (Access Care), UNC-Chapel Hill, Orange County Health Department, and Chatham County Health Department. This evidenced-based medical home model aims to help close the gap in the health status of minorities who have diabetes and heart disease. The Chatham County Public Health Department’s role is to continue its efforts in providing diabetes education services for all Chatham County residents.

 

                     

Discussion & Analysis: The Chatham County Public Health Department (CCPHD) will

do the following:

                     Offer 20 diabetes education group classes (DSME)

                     75% of patients who complete class (10 hours of education) will have a A1C of 7% or less at 3 month follow-up, check feet daily, & B/P of < than 130/80.

                     Track # of referrals to DSME, # seen, # who completed, demographics, and reason for referral.

                     Offer scholarships and/or incentives to patients for actively participating in the classes in hopes of including and retaining patients who have the inability to pay (reimbursed through the grant).

                     Educate medical home providers and other community partners on referral process at least 2 during the year at meetings.

                     CCPHD will attend at least 4 health fairs of community events for targeted outreach during the year; participate in at least 4 partnership meetings during the year and agrees to cross refer to partner classes and provide 2 Living Healthy classes each grant year.

 

                     

Budgetary Impact: No Local Funding Required.
