Abstract
Action Requested: Vote on a request to accept $4,160 Healthy Communities Grant

Introduction & Background: These are additional funds from the Healthy Communities Grant Program, in addition to the $26,831 previously accepted. The Healthy Communities Program seeks to address the risk factors of physical inactivity, poor nutrition, tobacco use, violence and unintentional injury. Research shows that implementing systems and environmental changes can result in positive behavior changes that decrease chronic diseases and injuries and improve health. Examples of such broad-based strategies include providing access to nutritious foods, improving safe options for active transportation, promoting tobacco free facilities, supporting diabetes management programs, and providing evidence-based practices and interventions for violence and injury prevention in communities.
The Healthy Communities Program provides funding for county and district health departments to develop and implement community-based initiatives to create policies and environments that support increased physical activity, promote healthy eating, reduce obesity, prevent the use of tobacco, support diabetes self-management and prevent violence and injury.

Discussion & Analysis: Based on the approved State Fiscal Year 2015-16 Action Plan, accomplish one of the following:
Increase the number of new interventions implemented
Expand the reach of existing interventions by implementing them in at least one additional setting or location.
Provide and/or attend one training opportunity for staff and community partners to support the successful implementation of interventions, on one of these topics:
Healthy Equity and Health Disparities, Program Evaluation and Quality Improvement and Evidence-based interventions for addressing chronic disease and associated risk factors.
