Action Requested: Vote on a request to approve $2,000 BCBS Funds
Introduction & Background: Blue Cross Blue Shield Foundation has developed a process for a technical assistance opportunity for partners to join Communities Joined in Action (CJA) as members. In addition to membership in CJA, BCBS is able to provide $1,000 stipends for up to two participants from each partnership to support registration and travel expenses for the annual conference. As a component of the conference, the BCBSNC Foundation is partnering with the Louisiana Public Health Institute to offer a pre-conference “affinity” session on Community-Centered Health Homes (CCHH). Developed specifically for NC grantees, as well as for five community health centers in Gulf-region states that are implementing CCHH as part of a demonstration project. The purpose of this session is to connect with other organizations and communities working to implement CCHH.
Discussion & Analysis: Each year, Communities Joined in Action brings together health stakeholders from across the country to share emerging lessons and innovations to improve our health care delivery system and to take actions that improve health in our communities. The 2015 conference will include compelling plenary presentations by national leaders, large and small group discussions of current challenges and opportunities, and focused workshops to share and discuss local, regional, and national innovations. The conference is most appropriate for members of community based organizations and coalitions, healthcare providers, hospitals and health systems, community health centers, free clinics, donated care networks, public health professionals and anyone interested in improving the health of underserved communities.
