Abstract
Introduction & Background: The Chatham County Public Health Department (CCPHD) is proposing the purchase of a mobile health clinic to expand access to healthcare services throughout Chatham County while also providing a flexible resource that can support the County during emergencies and other community needs.
CCPHD has explored the use of a mobile healthcare unit for several years, with planning beginning in earnest during the COVID-19 pandemic and vaccination response. The need for mobile healthcare continues due to transportation barriers, the geographic size of Chatham County, and the opportunity to increase participation in services by bringing healthcare directly into communities. The 2024 Chatham County Community Assessment identified Access to Healthcare Options as one of the County's three Health Impact Priorities. More than one-third of surveyed households identified healthcare as an area the County should focus on to improve health and quality of life, while community focus groups also identified the distance residents must travel to receive healthcare as a concern.
The mobile clinic would allow CCPHD to provide services in areas throughout the county rather than relying exclusively on residents traveling to existing clinic locations.
Discussion & Analysis: CCPHD has begun identifying locations and community partners for regular deployment of the mobile clinic. More than 34 potential locations have been identified, with initial approval received from more than 10 community partners, including County Parks and other community locations. During the first six months of operation, CCPHD anticipates deploying the unit at least monthly, with the longer-term goal of twice-monthly deployments in addition to community events and other scheduled activities.
Services provided through the mobile clinic are anticipated to include well-child assessments, immunizations and seasonal flu clinics, sexually transmitted infection testing, behavioral health services, health education, and other core public health clinical services. The unit would be staffed primarily with existing CCPHD personnel based on the services being offered at each deployment.
The proposed unit would include one to two examination rooms, laboratory space, an intake/interview area, restroom facilities, wheelchair/ADA access, and flexible workspace that can be used for coordination and support functions.
While the primary purpose of the unit is to expand access to Public Health services, its design also provides the County with additional capabilities during emergencies and other community events. The unit could be deployed as a field clinic following severe weather or other emergencies, support emergency shelters, provide vaccination or other healthcare services during an emergency, and provide workspace for coordination activities. This multi-use approach increases the overall value and utilization of the asset.
Staff evaluated both motorized RV-style units and towable trailers. A trailer platform is recommended because it provides greater operational flexibility and resiliency. A mechanical failure involving a motorized unit could make the entire mobile clinic unavailable. A trailer can be moved using compatible tow vehicles, reducing dependence on a single vehicle or powertrain while allowing the County to replace or substitute the tow vehicles when necessary. The trailer configuration also provides the space necessary for clinical services, restroom facilities, ADA accessibility, and other support functions.
The proposed unit is priced at approximately $250,000. Staff reviewed other available mobile medical units and found comparable alternatives at substantially higher costs, including used units priced at approximately $350,000 and new units ranging from approximately $600,000 to $1 million depending on size and configuration. Based on the proposed configuration, intended uses, and comparison with available alternatives, staff believe the proposed unit provides a cost-effective option for expanding mobile healthcare services in Chatham County.

Ensure public safety through adequate resources, plans, and partnerships to keep people and property safe.
Budgetary Impact: The total purchase price is $250,000. Grant funding is anticipated to be applied toward the purchase, subject to final approval by the grant funder. The remaining cost will be funded from previously appropriated County funds; therefore, no additional General Fund appropriation is requested as part of this action.
